Team Book Order

Each Head Football Coach should have the following information in a notebook available for all book checks. 

A copy of the team's certified roster.

A copy of the team's Mandatory Play Roster.  

All Coaches USA Football Heads Up Football Training Certificate in the back of the book.

BOOK ORDER REVISED AUGUST 3, 2023

For every player on the roster, the following information is required and required to be in the exact order as listed below for all book checks:

Sleeve 1     Front    I.D. Cards / Proof of Age

Sleeve 2     Front    Physical Fitness Certification dated after 1/1/2023

                   Back    Medical History

Sleeve 3     Front    Participant Contract                

                   Back     Parental Consent
